# Alternative Therapy For Bladder Cancer

## Intravesical Bacillus Calmette-Guerin (BCG) Therapy

 BCG treatment is an intravesical immunotherapy – meaning a liquid drug that’s administered via catheter to activate immune system cells that target abnormal bladder cancer cells and eliminate them. BCG is typically reserved for early stage nonmuscle-invasive bladder cancer which still exists close to or on the inner lining. Patients diagnosed with muscle-invasive cancer face an increased risk of distant metastases that could potentially prove lethal.

 Most advanced bladder cancer, both MIBC and metastatic disease, is usually best addressed using systemic therapy – drugs that circulate throughout the body to reach cancer cells – combined with local treatments like surgery or radiation.

 Chemotherapy is the foundation of systemic treatment for bladder cancer, with numerous regimens including those using cisplatin-based combination therapies having the best documented outcomes. Neoadjuvant chemotherapy with these agents greatly improved survival for MIBC as did radiotherapy; for unresectable or metastatic tumors chemotherapy remains the cornerstone treatment option and newer immunotherapies such as immunotherapy may prove more successful;

## Chemotherapy

 Cancer-fighting drugs can enter your body through various methods. They can enter via an intravenous line (intravesical chemotherapy), directly into the bladder (intravesical chemotherapy), the abdomen (intraperitoneal chemotherapy), or your chest cavity (intrathecal chemotherapy). Or they could even be given intravenously into your bloodstream (intravenous chemotherapy).

 Immunotherapy helps the immune system detect and eliminate tumor cells more effectively. A type called checkpoint inhibitor blocks a protein on cancer cell surfaces so your own immune system can recognize them more readily – for instance pembrolizumab (Keytruda).

 Muscle-invasive bladder cancer can spread beyond its original boundaries and into surrounding tissues, making treatment more challenging than earlier-stage cancers. Surgery is often recommended to reduce the risk of the cancer returning; depending on your specific circumstances, surgery might involve having all or part of your bladder removed (radical cystectomy) while other therapies may reduce chances that new cancers form or return.

 After having your bladder surgically removed, chemotherapy can help eradicate any cancer cells remaining. Intravenous chemotherapy is usually given through a vein; however, medicines can also be delivered directly into your bladder via catheterization or to areas in your belly where cancer was located (intraperitoneal and intrathecal chemotherapy).

 Immunotherapeutic options provide additional help to your immune system as it battles cancer more effectively, both alone and when used alongside BCG therapy. Some of these therapies have even become standard of care in cancer care regimens.

## Radiation

 Additional treatments that may be employed to address invasive bladder cancer include multiple doses of intravenous chemotherapy, radiation, and immunotherapy. These therapies may be given along with BCG or after it, or they can even be given for those whose cancer returns post surgery.

 Radical cystectomy refers to surgical removal of an entire bladder. It may be advised for people who are either ineligible for traditional surgery or their cancer keeps coming back despite BCG and other treatments, as this option might require finding another means of urination such as using ureal conduits or creating an alternative neobladder from part of the intestine as their only options for relief.

 Research efforts are also ongoing to discover new immunotherapeutic treatments for both invasive and non-invasive bladder tumors, with some promising findings so far.
