Pulsed Electro-Magnetic Field (PEMF) Therapy

Only three studies were identified that compare PEMF with sham treatments for pain relief in osteoarthritis patients and found no statistically significant difference in pain reduction between groups; however, interpretation should be undertaken with caution as quality data available is limited and only one research project examined PEMF effects on function in knee osteoarthritis patients.

PEMF must be administered at an optimal frequency, intensity and application duration to be effective. When applied too long at high frequencies it may interfere with natural healing by encouraging cell proliferation and inhibiting apoptosis; shorter applications at lower frequencies have shown to increase biomechanical strength early in wound closure processes; further research must be conducted into ideal PEMF parameters to develop standard experimental guidelines for future controlled trials.

Static Magnet Therapy (SMF)

Static magnetic field (SMF) therapy is a fundamental form of electromagnetic therapy, serving as the basis for other forms of field therapy. Magnetic induction intensities range from low (5mT) to very strong (>1T).

Chronic SMF exposure has been demonstrated to have an adaptive response on microvasculature, returning it back to normalization of dysfunctional vasculature (9-10). Furthermore, multiple in vitro studies have demonstrated how SMF can influence mesenchymal stem cell differentiation into osteoblasts and osteocytes (17,18).

Studies of SMF therapy on animal models have yielded promising results. For instance, Xu et al. implanted 180 mT SMF magnet material onto the lumbar spine of OVX rats and observed an increase in bone mineral density near magnet compared to untreated controls.

Another study demonstrated that 0.4 T SMF enhanced matrix vesicle secretion from human umbilical cord mesenchymal stem cells while simultaneously decreasing their proliferation rate and increasing Osteocalcin and OCT-4 gene expression levels associated with bone formation.

Animal studies demonstrate the efficacy of static magnetic field therapy; however, few human trials have been performed to demonstrate its efficacy. Of the RCTs conducted thus far, none has demonstrated significant differences in pain levels, SSS or FSS scores, or Patients’ Clinical Global Impression of Change between static magnetic field therapy and placebo treatments.

A quality assessment (QA) score was devised to assess the level of detail provided by authors when discussing 10 essential dosing parameters, including magnet characteristics (materials, dimensions and pole configuration); SMF application (frequency, duration, site of application and support device); target tissue distance from applied permanent magnet; and target distance. Of the 10 essential parameters described by authors only two were fully described while eight others either partially or not at all made it impossible for investigators to replicate study protocols.

Once these methodological issues have been resolved, more expensive clinical studies using SMF will likely fail to show any discernible benefits; although anecdotal feedback from champion athletes worldwide suggests that static magnet therapy could indeed help in relieving pain and improving soft tissue repair.
