Gastric Torsion Medicine For Dogs

GDV, also known as gastric dilatation volvulus (GDV), usually affects large breeds with deep chests and requires immediate surgical intervention to avoid imminent death. If left untreated, surgery could result in GDV being fatal for the animal.

Causes

Gastric dilatation volvulus (GDV), more often than not affecting dogs but less frequently cats, guinea pigs, and rabbits alike, occurs when the stomach becomes distended with air or food and then rotates on itself, restricting blood flow through its walls and leading to tissue death and systemic complications. Progressive distention compresses major abdominal vessels preventing venous return, decreasing oxygen delivery across organs like the heart lungs kidneys liver. Diminished respiratory output leads to hypoxia while hypovolemia causes shock.

GDV occurs when gas becomes trapped inside of the stomach due to rotation and dilation; x-rays of GDV will typically show a double bubble pattern due to gas entering through dilation of the stomach, often from primary laxity of either gastrocolic ligament, diaphragmatic hernia of stomach, peptic ulcers, neoplasia or foreign body ingestion. GDV can be caused by primary laxity of either gastrocolic ligament, diaphragmatic hernia of stomach dilatation due to dilatation of stomach dilatation due to dilatation resulting from dilatational dilatation of stomach dilatation with gas being held back. Early volume resuscitation and decompression is of primary concern; orogastric tubes are often used to reposition the stomach in this regard during surgery under general anesthesia; prior to placement an area caudal to the last rib and ventral to the transverse vertebral process should be clipped off for aseptic preparation – this helps avoid accidental puncturing of overlying spleen – before inserting smooth-surfaced orogastric tubes are placed for successful volume resuscitation and decompression.

Once repositioned, the stomach should be gently repositioned by applying downward pressure to its fundus and gentle traction on its pylorus in order to facilitate counterclockwise rotation. Careful evaluation is conducted of both stomach and spleen for any irreversible vascular compromise (black, grey/green or thin palpation on palpation); if implicated spleen may need to be removed. Once the stomach has been permanently attached back onto abdominal wall via gastropexy (permanent affixation), in order to avoid volvulus in future years.

During surgery, a constant supply of oxygen should be supplied in order for surgeons to observe for signs of gastrointestinal pulsation and help avoid aspiration of lung tissue into the pleural space. Halothane or isoflurane should be used for anesthesia purposes.

Once the stomach has been repositioned and evaluated, it must be sutured into place. Two rows of stay sutures are typically applied in simple continuous fashion; typically one line lies along the deep edge of the gastric body incision opposite to where craniodorsal aspect of gastropexy incision lies; second row is along superficial or ventral aspect of body incision using typically long-acting monofilament absorbable sutures with an absorbable monofilament thread of 2-0 monofilament yarn is placed along first row and first row opposite craniodorsal aspect; second line placed where craniodorsal aspect meets gastropexy incision. 2-0 monofilament long-acting absorbable monofilament long-lasting absorbable sutures with long-actance.
