Sacred sound vibrations or frequencies (created by chanting, drums, gongs, chimes, tuning forks, singing bowls and more) have been valued across cultures for millennia for their healing powers and ability to promote balance, restoration, and well-being. Emerging scientific research appears to validate many of these frequencies, and the use of sound healing has become an increasingly popular alternative health modality for stress reduction and overall wellness.

For this study, a total of 30 patients with an altered state of consciousness underwent a clinical trial that involved three music therapy sessions during their hospital stay. During each session, fNIRS was used to monitor the patients’ brain activity and response to musical stimuli. Participants were split into a low frequency group, a mid-frequency group, and a high-frequency group. The frequency range for each group was 0.5 Hz to 3.5 kHz. Fetal Vibroacoustic Stimulation

Fetal vibroacoustic stimulation is a popular test used during tests of fetal well being such as a non-stress test. It is hoped that it will reduce the number of false positive cardiotocography readings without affecting their predictive reliability.

Two trials (Marden 1997; Sood 2007) compared fetal vibroacoustic stimulation with mock stimulation. The method of allocation concealment varied between trials. In one trial the results were interpreted blindly by an independent perinatologist.

Fetal heart rate (FHR) reactivity

Fetal heart rate (FHR) reactivity tests are a non-invasive test that can detect changes in the fetal heartbeat. The test uses a hand-held device that emits brief sounds through the mother’s abdomen to wake up the baby. A reassuring FHR pattern is an indicator that the baby is well. However, a non-reassuring pattern may be due to the baby’s sleep period, which can alter results. If the reactivity test is not reliable, doctors will have to perform an internal monitoring test to verify the status of the baby.

Internal monitoring is an invasive procedure that requires the rupture of the membranes, dilation of the cervix, and access to the fetal presenting part. In addition, it requires a high degree of training to interpret the results. Moreover, the equipment is expensive and may require frequent maintenance. Therefore, a simple, inexpensive test would be ideal. A vibroacoustic stimulation test can provide reassurance of fetal health in the presence of a non-reassuring CTG.

The reactivity of the fetal heart rate can be a useful predictor of the onset of labor and fetal distress. However, the evidence from randomised controlled trials is not conclusive. In one trial, women were randomly allocated to either a 5 second vibroacoustic stimulation or a mock stimulus and interpreted blinded by an investigator who did not know the group allocation. The results showed no difference in the number of fetal heart rate accelerations or decelerations, modified biophysical profiles, caesarean section for fetal distress, umbilical arterial pH, and Apgar scores at 1 and 5 minutes after birth.

In another study, pregnant women were assigned to a control or treatment group using computer-generated random numbers. The number of participants was limited to 30 per group. The participants were examined in a quiet room and in semi-recumbent position to minimise the risk of supine hypotension. The fetal heart rate was monitored with Corometrics 115 monitors. The vibratory acoustic stimulator used in the study had a frequency of 75 Hz and a sound intensity of 110 db at 1 m in air. The tracings were analysed by an independent perinatologist who was unaware of the groups’ assignments.

Fetal heart rate (FHR) non-reactivity

The non-reactivity of the fetal heart rate (FHR) during the vibroacoustic stimulation test can be a useful indicator that the fetus is not in danger. However, there is a lack of randomised controlled trials that validate this test as an effective tool in early labour and antenatal care. Despite this, several studies have shown that the use of vibroacoustic stimuli may be beneficial for both maternal and fetal wellbeing. In addition, the test has the advantage of being simple and non-invasive. Other tests, such as fetal scalp blood sampling and fetal oximetry, require dilation of the cervix, rupture of the membranes, and access to the fetal presenting part. This can be difficult in labour, especially in case of a traumatic vaginal birth or cervical dystocia (East 2008).

The study by Polzin and colleagues investigated the effects of vibrational stimuli on fetal heart rate during the paced breathing test and during adjacent recovery periods. The authors found that the fetal heart rate accelerations during the paced breathing test were significantly higher than those of the resting period. They also found that the fetal heart rate accelerations were more consistent during the paced breathing test than in the resting period. This suggests that a consistent physiologic response to the stimuli is important for an accurate interpretation of the fetal heart rate pattern during the paced breathing test.

For the purposes of this trial, the obstetricians assigned 202 pregnant women at term in vertex presentation with cervical dilatation 4 cm and intact membranes to either the control group or the acoustic stimulation group. The women were randomly assigned to the group using a computerised number generator. The acoustic stimulation was performed by placing an electronic vibroacoustic stimulator on the mother’s abdomen, in the region of the fetal head, with a sound intensity of 75 db and frequency of 80 Hz. The acoustic stimulus was applied for 3 seconds and repeated up to 3 times at 1-minute intervals if a non-reactive fetal heart rate pattern was not observed within 40 minutes.

The acoustic stimulator was placed over the woman’s abdomen, and the fetal heart rate (FHR) was monitored by a telemetric fetal monitor. The tracings were interpreted blindly by an experienced perinatologist.

Fetal heart rate (FHR) postdatism

Fetal heart rate (FHR) decelerations are an indication of a potentially compromised fetus. They can occur during a nonstress test (NST) or during labor and delivery. In a study of 2000 NSTs conducted on high risk pregnancies, variable FHR decelerations occurred in 55.3% of the cases. In addition, these decelerations are associated with abnormal cord position and oligohydramnios. In the presence of these fetal signs, it is important to minimize labor induction in order to avoid perinatal morbidity. However, it is also important to recognize that not all nonstress tests are positive and that a low 5-minute Apgar score does not necessarily mean fetal distress.

The use of a vibroacoustic stimulation test may reduce the incidence of postdatism in patients who are at a high risk for preterm birth. This method is less invasive than the contraction stress test and does not interfere with the normal physiological process of labour. It is also inexpensive and easy to interpret. Moreover, it is not a substitute for a biophysical profile, which should be performed before the onset of labor.

A variety of different vibroacoustic stimulation techniques have been used in antepartum testing. The sound frequency, intensity, and duration of the stimulus are all factors that affect fetal response to the stimulation. However, the results of these studies have been conflicting.

In a recent trial, a randomized controlled trial compared the use of VAS with a biophysical profile alone in pregnant women at a high risk of preterm labor. The study involved 715 women of at least 28 weeks gestation who were randomly assigned to either the VAS or control group. The BPPS was assessed for 30 minutes in both groups. If the BPPS was not reassuring, the woman was given a 3-second stimulation with the VAS device. The BPPS was reassessed for another 30 minutes.

The results showed that the use of VAS reduced the number of women who needed a cesarean section and had a low 5-minute Apgar score. The incidence of respiratory complications was also reduced in the VAS group. However, there were too few deaths to determine a difference between the two groups.

Fetal heart rate (FHR) post-stress

The vibroacoustic stimulation test is a non-invasive test used to increase the accuracy of antepartum cardiotocography (CTG) monitoring. It uses a transabdominal vibration to stimulate the fetal heart rate and elicit a response, which is then interpreted as an indication of fetal well-being. This method can significantly shorten the test’s testing time and reduce maternal anxiety. However, non-reactive cardiotocography tracings are still a significant cause of concern for maternity services and should be monitored closely in case of any abnormalities.

In a number of studies, a significant percentage of FHR accelerations and transient tachycardia recorded following a VAS are found to be false positives. This is thought to be due to the presence of stress in the mother that can induce a disproportionate response from the fetal heart rate (FHR) resulting in a false signal. This is especially the case when a woman is experiencing a stressful pregnancy, such as depression. It is also suggested that women experiencing depression are more likely to experience a prolonged onset of FHR acceleration, which can fuse into a sustained tachycardia and be misinterpreted as an adverse event.

While some trials have shown that a VAS can improve the efficiency of antepartum cardiotocography, more research is needed to determine whether this improvement in test accuracy is clinically meaningful. For example, the VAS has not been shown to decrease the occurrence of intrauterine death, caesarean section for fetal distress or admission to the neonatal intensive care unit.

Currently, there are many methods of assessing a baby’s sensitivity to labour, including invasive blood sampling and fetal oximetry. However, these are costly and require a dilated cervix and access to the fetal presenting part. It would be useful to develop a test that can detect a baby’s sensitivity to labour without the need for these invasive procedures.

One such technique is the fetal vibroacoustic stimulation test, which involves an ultrasound device that vibrates the uterus to produce a sound stimulus to the fetus. The fetus responds to the stimulus by accelerating its heart rate, which is then interpreted by a perinatologist. The method is simple and easy to use, which may make it an attractive alternative to invasive methods of detecting fetal distress.
